Every charity is different and some of the smaller ones, such as Fragile X, do not always have such forms.

If they do, you'll get them.

If they dont, simply make up your own.

They aren't strictly neccessary

You walk up to people and say:

"I know where you live, I know where your family live, i know where your friends live. i race C90's, i have influential 'friends' , I am not a person to cross, you really need to sponsor me,right?"

Then look them in the eye, raise an eyebrow In Roger Moore style and in true ND style , smile.

If they don't sponsor you, then return here with details and thier homes will be demolished, thier souls torn from thier bodies and penalty points on thier Driving Licences shall appear.
